How to fill in Form 4A, box by box

Before you start
Form 4A is the prescribed notice for proposing a new rent under Section 13 on a periodic assured tenancy in England. Before you fill it in, have these to hand:
- The current rent, the new rent, and how often it's paid.
- The date the tenancy started, and the date of the last increase (if there's been one).
- The full name of every tenant on the agreement, and the landlord's name and contact address.
Remember the ground rules: you can increase the rent once every 12 months, with at least two months' notice, and the new rent must start on the first day of a tenancy period (for a monthly tenancy, your rent day).
Section 1 — the tenant and the property
1.1 Name(s) of tenant(s). Name every tenant on the tenancy agreement. In a joint tenancy, leaving one off can make the notice invalid.
1.2 Address of the premises. The address of the property the tenancy relates to — as on the agreement.
Section 2 — the landlord (including companies)
2.1 Name of the landlord. If the property is jointly owned, name every landlord. If the landlord is a company (for example a limited company), put the company's name here — it is the landlord. A director or other authorised person signs on its behalf at Section 5 (see below).
2.2 Contact address. The address the tenant can use to contact you or serve documents. It must be complete.
2.3 Phone and email. Optional, but recommended. Note: if you give an email address, you're agreeing the tenant and the tribunal may serve documents on you by email.
Section 3 — the agent (only if one is acting)
Complete Section 3 only if a letting or managing agent is acting for you. If you're handling the increase yourself, leave it blank.
Section 4 — the rent and the dates (where most mistakes happen)
- 4.1 Current rent and how often it's paid (for a monthly tenancy, "per month").
- 4.2 The date the tenancy started.
- 4.3 The most recent increase (if any). This sets the once-every-12-months clock.
- 4.4 The first rent increase after 11 February 2003. This feeds a 52-vs-53-week rule that only affects sub-monthly tenancies (e.g. weekly) — for a monthly tenancy it doesn't change your earliest valid date. Enter the date if there's been an increase since then; if there hasn't, leave it blank.
- 4.5 The proposed new rent.
- 4.6 The date the new rent starts. At least two months after the notice is served, and on the start of a tenancy period. This is the box that voids most notices — see what makes a notice invalid.
- 4.7 Charges. Enter "nil" in each box unless the rent genuinely includes a separately identified charge for council tax, water, or a fixed service charge.
Section 5 — signing it (and signing for a company)
You (or your agent) must sign and date the notice. A typed or electronic signature is fine — the form's own guidance allows it.
Joint landlords: each can sign, or one can sign on behalf of all (with their agreement), while Section 2.1 still names everyone.
A company landlord: an authorised person — usually a director — signs. Put their name as the signature, state their capacity (e.g. "Director") in the Print name line, and tick "Landlord" (not "Landlord's Agent", which is for a separate letting or managing agent). The company is already named as the landlord at Section 2.1.

Do I have to name every tenant on Form 4A?

My landlord is a limited company — whose name goes on the form?
The company is the landlord, so its name goes in Section 2.1. A director or other authorised person signs on its behalf at Section 5, with their capacity (e.g. Director) noted in the print-name line.
What date do I put for the new rent to start?
